Pilots generally monitor a set frequency when over large stretches of water so they can communicate with each other. It often gets abused in the real world.

Lets say you just switched over or coming to an island.

    You would say

Blabla Center, Callsign with you, FL<Insert crusing here>.

In real time, with a sample I used on vatsim today.

Cleveland Center, SWA7362 with you, FL320.

    Its pretty simple and straightforward.

HF (High Frequency) is used. Some SATCOMM available soon and possibly to be tied up with a thing called ADS-B.

HF has further range than the domestic VHF. HF is very crackly and sometimes itself impossible to understand due to quality so pilots use a system called SELCAL of selective-call which alrets them each time a message needs to be passed.

    SELCAL: http://en.wikipedia.org/wiki/Selcall

    ADS-B: http://en.wikipedia.org/wiki/Automatic_dependent_surveillance-broadcast
